It is important to have the entire extremity relaxed when making an assessment of a patient's knee extension. Hyperextension degrees are measured in negative "-" degrees, while a lack of full extension is measured in positive "+" degrees.

In patients who lack full extension, the examiner can place their hand under the knee and ask the patient to attempt to push their knee against you to help determine the extent of their extension deficit.

Gentle pressure and support to the anterior leg helps to get a patient to relax to assess maximal knee flexion.
Comparison of the amount of flexion in the symtomatic and the normal contralateral knee.
